Emergencies

Call 911 for critical emergencies. If you need to talk to one of our doctors after hours call (734) 712-1400. You will reach an operator who will page the doctor on call. If your call is not returned within 15 minutes, call back and inform the operator.

Appointments

• How do I see one of your doctors?

  Your primary Care Provider will refer you to our office.

• How do I set up/cancel/change an appointment?

  Call our office at (734) 712-1400 during business hours and our staff will assist you.

• Do I need a referral form at the time of my visit?

  Depending on your insurance company, you may need a referral at the time of your visit. Check with your Primary Care Physician's office.

• What should I bring with me?

  Your insurance card(s) and photo identification

  HMO referral, if required by your insurance company

  Any lab test results, medical records, xrays, etc, pertaining to your past/present medical condition to help your physician and avoid unnecessary repetition of testing.


Insurance/Payment

• What do I do if my insurance has changed?

  Call our office at (800) 772-8147 with any changes.

• How much is my co-pay?

  Depending on your insurance company, your co-pay (the amount you must pay) usually ranges from $5-$50 per office visit. Call your insurance company to determine how much you may be responsible for including deductibles. 

• Which insurance plans do you accept?

  The most common insurance plans are listed in the "About Us" section under "Insurance & Affiliations".  If your insurance is not on that list, please call our billing department at (800) 772-8147 to see if we currently participate with your insurance plan. • Will I need to pay when I come in for my appointment?

  Yes, co-payments are expected at the time of your visit.

Prescriptions

• Who do I contact for a prescription refill?

  Call the office at (734) 712-1400 during regular business hours. If it is after hours and you are unable to wait until regular business hours for a renewal, call the office at (734) 712-1400. You will reach an operator who will page the doctor on call. If your call is not returned within 15 minutes, call back and inform the operator.

• Will you call me with test results?

  If there are significant abnormalities on any of your tests, your doctor or someone from our office will contact you and/or your primary care provider as soon as test results are available.
  Otherwise, your doctor will discuss the results of the testing at your next office visit, or the results of the tests will be mailed to you.
