How do I see one of your doctors?Your primary Care Provider will refer you to our office.

How do I set up/cancel/change an appointment?Call the appropriate office and our staff will assist you.

Do I need a referral form at the time of my visit?Depending on your insurance company, you may need to bring a referral at the time of your visit. Check with your Primary Care Physician's office. Referrals may be faxed to our billing department at (800) 772-8147

What should I bring with me?
Your insurance card and referral form (if necessary, see above).HMO referral, if necessary.Any lab test results, medical records, xrays, etc, pertaining to your past/present medical condition to help your physician and avoid unnecessary repetition of testing.

How much is my co-pay?
Depending on your insurance company, your co-pay (the amount you must pay yourself) usually ranges from $5-$20 per office visit. Call your insurance company to determine how much you will have to pay.

Will I need to pay when I come in for my appointment?
Yes, you are expected to pay at the time of your visit. We accept cash, checks, Visa, MasterCard, and Discover.
