Call 911 for critical emergencies. If you need to talk to one of our doctors after hours call (734) 712-8100. You will reach an operator who will page one of our doctors. If your call is not returned within 10 minutes, call (734) 712-3888 and inform the hospital operator.

Do I need a referral form at the time of my visit?Depending on your insurance company, you may need to bring a referral at the time of your visit. Check with your Primary Care Physician's office. Referrals may be faxed to our billing department at (734) 712-8112

What should I bring with me?
Your insurance card and referral form (if necessary, see above).HMO referral, if necessary.Any lab test results, medical records, xrays, etc, pertaining to your past/present medical condition to help your physician and avoid unnecessary repetition of testing.

How much is my co-pay?
Depending on your insurance company, your co-pay (the amount you must pay yourself) usually ranges from $5-$20 per office visit. Call your insurance company to determine how much you will have to pay.
