Do I need a referral form at the time of my visit?
Depending on your insurance company, you may need to bring a referral at the time of your visit. Check with your Primary Care Physician's office.

What should I bring with me?
Your insurance card, copy of photo id (such as your drivers license), a referral form (if necessary, see above), and your completed NP packet if you received one in the mail, by fax, or from our website.
